As mulheres entre 26 e 40 anos podem ganhar um importante aliado no combate ao HPV, um vírus transmitido pelo contato sexual, líder em infecção e principal responsável pelo câncer de colo de útero.
A Agência Nacional de Vigilância Sanitária (Anvisa) estuda liberar a aplicação da vacina contra a doença para este grupo etário. Por enquanto, o único público que tem autorização sanitária no País para receber a imunização é o de jovens do sexo feminino, de 9 a 25 anos. As doses –são três, no total, para garantir a eficácia – só são oferecidas na rede privada de saúde, por preços salgados: as três doses ultrapassam mil reais.
O pedido de ampliação de público foi feito pelo produtor da vacina, o laboratório Merck Sharp. Segundo os responsáveis pela empresa, estudos atestaram que a mesma dosagem da vacina contra o HPV aplicada em adolescentes e jovens é eficaz na mulher adulta.No total, 12 países já estão aplicando as doses nas mais velhas, entre eles Chile, Canadá e Austrália.
Um dossiê foi encaminhado à Anvisa informando sobre a segurança de aplicação em mulheres mais velhas’, afirmou o ginecologista Nelson Vespa, responsável da Merck no Brasil pelas pesquisas com a vacina.
O especialista explica que o dossiê foi baseado nos resultados de um estudo internacional feito com 3.819 mulheres entre 24 e 45 anos, com duração de quatro anos. Ele mostrou que, também neste público, a vacina é eficaz para prevenir: a infecção persistente, as lesões de baixo grau, as lesões pré-cancerígenas de alto risco para o colo do útero, a vagina e a vulva, além de lesões genitais externas e verrugas genitais. Se a mulher nunca teve contato com nenhum tipo de vírus do HPV – são mais de 100 existentes – a eficácia de proteção é de 91%. ‘Atestamos também que, caso a paciente já tenha sido contaminada, a vacina auxilia em prevenir a evolução da doença’, afirma Vespa.
Além da Merck, o laboratório GSK produz uma outra vacina contra o HPV de eficácia já confirmada pelas autoridades sanitárias. Esta imunização também só está disponível na rede particular de saúde e para pessoas do sexo feminino entre 10 e 25 anos.
A Anvisa informou que já analisou o pedido de extensão etária para aplicação da vacina feito pela Merck. A Agência solicitou mais informações ao fabricante. A expectativa é que a aprovação – ou não – saia ainda este ano.
Uma coleção de casos
A infecção pelo HPV é uma doença transmitida pelo sexo sem proteção, que pode afetar homens e mulheres e, se não tratada, pode evoluir para o câncer de colo de útero. Na maior parte dos casos não há sintomas. Quando ocorrem, eles são caracterizados por verrugas ou manchas brancas na área genital. O exame principal para a detecção da doença nas mulheres é o papanicolaou.
Apesar de não existir um levantamento consolidado dos casos do chamado papiloma-vírus humano (nome científico do HPV) no País, os médicos estimam que a doença já afetou – ou vai afetar – 75% da população sexualmente ativa do País. Um estudo recém publicado pela Secretaria de Estado da Saúde de São Paulo avaliou os 56.803 casos de DSTs notificados entre janeiro de 2007 e junho de 2009 e constatou que uma em cada três registros era relacionado ao HPV (32,6%).
As projeções do Instituto Nacional do Câncer (Inca) também reforçam a perigosa disseminação do papiloma-vírus humano. Até o final deste ano, o Brasil vai acumular 18.430 novos casos de câncer de cólo de útero. Ele é o segundo tipo de câncer que mais mata a população feminina, atrás apenas da neoplasia de mamas.
Vacinas na rede pública
A perigosa disseminação de casos de HPV fez com que um grupo de especialistas de reunisse para estudar a implantação da vacina no calendário público de imunização. No ano passado, por meio de verba repassada pelo Ministério de Ciência e Tecnologia, foi implantado o primeiro Instituto Nacional de Pesquisa em HPV.
Uma das missões da entidade é mapear a prevalência da doença entre os brasileiros e brasileiras e traçar estratégias para garantir as doses gratuitas. “Essa é uma meta muito importante para o Instituto do HPV, mas que ainda não foi abordada na prática. Nos próximos meses, esperamos determinar ações para disparar projetos que discutam a implantação das vacinas profiláticas contra o HPV na rede pública’, afirma a médica Luisa Lina Villa, diretora do Instituto e também pesquisadora do Instituto Internacional Ludwig de Pesquisa sobre o Câncer.
O Ministério da Saúde, o Inca e representantes dos departamentos de imunização de todo País também formaram uma câmara técnica para discutir a adoção da vacina na rede pública. O último posicionamento, afirmou o Inca, reconheceu a importância da vacinação gratuita – em especial para o público adolescente – mas alega que primeiro é preciso ter noção mais clara sobre a incidência do vírus HPV no Brasil.
Isso porque as vacinas existentes só protegem contra quatro dos mais de 100 tipos de vírus HPV. Ainda que os contemplados pela imunização sejam os mais comuns, não há um estudo sobre qual deles é o que mais circula no Brasil. Outra limitação apontada pelas autoridades brasileiras é o custo das doses.
‘A introdução da vacina na rede pública significaria um impacto de R$1,857 bilhão, apenas para cobertura da faixa etária de 11 a 12 anos. Como comparação, ressalta-se que o orçamento do Programa Nacional de Imunização é de R$ 750 milhões/ano’, afirma o grupo em documento.
Enquanto a vacina não entra no calendário público, ela pode ser adquirida na rede privada. Apesar disso os especialistas ressaltam que a melhor forma de prevenção ainda é o uso de preservativo em todas as relações sexuais.

Acho difícil, instiuir a vacina pelo PNI, sendo de alto custo e a prevenção por meio de preservativos mais barata, o que falta é ação educativa para efetiva-la.
